SuperWell® Disease and Maternity Management Program

Carolina Care Plan offers the SuperWell Disease and Maternity Management Program for our members who have been diagnosed with a chronic disease or who are currently pregnant to provide additional support and education that complements what they receive from their physician. Health Coaches work with the member to teach them how to avoid potential complications and the importance of complying with their prescribed treatment plan.

The SuperWell Disease and Maternity Management Program is available to eligible members with one or more of the following conditions:

• Asthma
• Chronic obstructive pulmonary disease
• Chronic pain conditions
• Congestive heart failure
• Coronary artery disease
• Depression
• Diabetes
• Pregnancy

When a member is enrolled in the SuperWell Disease and Maternity Management Program all comorbid conditions in the above list are addressed. To enroll one of your patients into the program, call us at 800/861-4826.
